Masks are now required during check in, while exiting the site, and while players are not actively participating.  They do not have to be worn during conditioning or competition.  See details below:

 

1.   Before players may participate, parents / players must fill out the usual AISD forms that can be found in Rank One.

 

Online forms are now live and ready on RankOne. Please get this link to your athletes and parents to complete. The sooner the better. There are 4 forms to fill out. “Yes” answers to the medical history page may require a full physical, but for now, one is not required.

2.       The symptom screening google doc is available below. This document must be completed daily, no exceptions, for players practicing on campus. Please complete this prior to arrival every day. We will also be checking temperature for everyone in attendance, this includes coaches.

3.       Players must bring their own water each day (no less than a gallon).  Players without water will not be allowed to participate.

 

 

Physical forms must be turned in and on line release forms completed before players of any level may practice.  The expiration date for physicals has been extended, so any physical used last year is still good.  Anyone who did not get a physical last year must obtain one from a doctor.  This is the only form to download; all other forms found in the Rank One portal are completed virtually.  The physical form (both pages) can be turned in when in person practice begins.

Follow the link below for all on line forms.  Click the "Electronic Participation Forms" tab. Then complete all three forms: AISD Participation, Emergency Card, and UIL Forms Signature Page.  The physical form that must be completed by a doctor is also available at this site.
